TMG IRIS Consultant Agency (ICA)
As Wisconsin’s first IRIS Consultant Agency, TMG partners with thousands of adults with disabilities and older adults statewide, empowering them to enjoy meaningful lives in their communities.
Since 2008, TMG has built a network of over 700 experts that support people enrolled in the IRIS program in all 72 Wisconsin counties. TMG’s knowledgeable, compassionate consultants become key allies in achieving long term care goals, and are happy to help the people we partner with live the life they choose.
Advocates4U
Advocates4U consultants are here to partner with IRIS participants to reach the goals they desire by exercising choice, control and freedom to design their own waiver eligible support and service plans to meet their functional, vocational, medical and social needs and to remain in the community.
Managed Care Organizations
When a person decides to enroll in Family Care, they become a member of a managed care organization. Managed care organizations operate the Family Care program and provide or coordinate services in the Family Care benefit package. Services are tailored to each individual’s needs, circumstances, and preferences.
Aging and Disability Resource Centers
Your local aging and disability resource center (ADRC) is the first place to go to get accurate, unbiased information on all aspects of life related to aging or living with a disability.

The ADRC provides information on a broad range of programs and services, helps people understand the various long-term care options available to them, helps people apply for programs and benefits, and serves as the access point for publicly funded long-term care.
